Avascular Necrosis: The bone Death

Avascular necrosis, also known as aseptic or ischemic necrosis, is a condition in which there is a loss of function of bone tissue. The affected bones become fragile and may fracture easily.

Avascular necrosis occurs when blood flow to part of the bone blocks. Without blood flow, the bone tissue dies from a lack of nutrients and oxygen. This causes a decrease in the mass of that area of the bone.

People usually develop avascular necrosis because of decreased blood supply to the bone. And this lack of blood supply damages the bone and cartilage. Usually, this reduced blood supply happens gradually over time because:

The blood vessels leading to the affected bone narrow from atherosclerosis (the buildup of cholesterol and other substances along artery walls)or other diseases

The blood vessels blocks or narrows because of a bone fracture

In some people, the cause is unknown. In these cases, it is- idiopathic avascular necrosis.

Post Covid Avascular Necrosis this is just because of heavy steroid dosages, during treatment of Covid.

Treatment of AVN

The primary line of treatment is Replacement arthroplasty. However, conventional procedures associate with significant complications and failure rates. Newer forms of arthroplasty using cementless components have been designed to reduce wear and pain; however, these newer techniques remain in the early phases of clinical development and long-term results are unknown. Therefore, there is a need to explore adjunctive options for improving the prognosis of patients with avascular necrosis of the hip.

The treatment’s primary aim is to prevent bone tissue loss, enhance joint movement and alleviate pain in the joints. The approach is simple at Sukhayu. We follow the following approach-

1. To stop the further progress of disease- immediately.
2. Ensuring the safety of other joints. When AVN is in right hip alone to confine it to the right side alone or vice-versa.
3. Avascular Necrosis can spread to other joints also- so to make sure that rest of your joints will be safe, is a prime approach.
4. To reduce the pain.
5. To resolve of the bone marrow edema.
6. Healing the fractures and bone decay.
7. Reversal of the Avascular Necrosis.

Vata controls movement. Every type of movement inside the body. This is Vata dosha which controls- carrying nutrition from one place to another place. Not only the nutrition. Excretion too.

Because nutrition is not reaching the destination and waste products are collected there. This is how bone death continues.

Pitta and Kapha are “immobile”. These don’t move from one place to another, without Vata. Therefore, this is Vata which is the most important factor for the disease.

But, Vata has Ether and Air only. It cannot cause blockage. The blockage comes from- Kapha dosha. Because of the muddy nature of the Kapha, it causes blockage.

When two out of three doshas are vitiated, how can Pitta not?

The truth is, in every disease all these three doshas disturb. This is just grading, which we need to know. From above it is clear that this is more about Vata than Kapha and least about Pitta.

Dhatus in Avascular Necrosis

Ayurveda talks about seven Dhatus. Out of these seven, it is thought that Avascular Necrosis is a condition of Asthi Dhatu (the bone). But it is not so. Avascular necrosis is not about the “asthi-dhatu” (bone). Bone is affected. Bone is not the cause behind the disease.

Just for the example- if there is the decay of muscle, due to neuropathy- what do you need to correct? Muscle or the nerves. Obviously nerves. The same applies to AVN too. The problem is – “Avasculature” (loss of blood supply). Not the bone. If it will be confined to bony tissue only, then this condition won’t afflict any other tissue. But it does so.

So this is all about the vasculature. The second Dhatu, from the Ayurvedic perspective. Anything and everything that moves inside the body is controlled by Vata. But without a carrier, nothing can move. The carrier here is- Rakta, which loosely translates into the blood. Yes, this is the problem.

What diet can help in Avascular Necrosis

Hence, this is a disease where the passage is blocked due to Kapha dosha which entraps the Vata. Vata moves strongly when it doesn’t get passage. Strong movements cause- destruction.

But in between, we need to make sure- regeneration of bone and reversal of avascular necrosis, therefore we worked more in detail.
